Item Health Coaching as an Intervention for Picky Eaters(2020-11-01) Matthews, SimonAlthough childhood “picky eating” or “fussy eating” or “food neophobia” is well established as a subject of research, commentary, and treatment guidelines, there is very little published research exploring the origins and basis of adult food neophobia, much less its treatment. Existing treatment guidelines for picky eating tend to focus on cognitive behavioral interventions. The consequences of picky eating, although not extensively researched, include inadequate nutrition and weight management difficulties—both significant contributors to the worldwide disease burden. Health coaching has a focus on personal choice, reflection on previous successes and current strengths, as well as identification of a personal health vision and achievable goals. As such, it may play a useful role in supporting behavioral change in adult picky eaters. A structured intervention, rooted in health coaching skills and culinary medicine aimed at supporting such change, is proposed.

Item Lifestyle Medicine Core Competencies: 2022 Update(2022-12-01) Rea, Brenda L.; Palma, Mechelle; Matthews, Simon; Kelly, John H.; Adamson, Karen; Lianov, Liana S.The lifestyle medicine core competencies were developed by a committee of physicians from several medical specialties to provide guidance on the knowledge and skills needed for physicians to provide high quality lifestyle interventions that optimize chronic disease outcomes. These competencies were published in the Journal of the American Medical Association (JAMA) in 2010 and used as the foundation for the first lifestyle medicine course and for the lifestyle medicine board certification examination. In the ensuing years, interest in the field and application has expanded to a variety of health professionals. With evolution of the lifestyle medicine evidence-base, the competencies have been updated. This article sums up the changes in their organization and content. Regular updates are anticipated to align with the ongoing scientific studies and evolution of the field.

Item Nudging as a Support for Behavioral Change in Lifestyle Medicine(2023-12-01) Matthews, SimonThe practice of lifestyle medicine and its emphasis on behavioral change continues to grow around the world. Yet much of the burden of disease weighing on healthcare systems from chronic, modifiable conditions remains stubbornly present. From a behavior change perspective, efforts to date have primarily focused on public health messaging and public health campaigns (global approaches) to interventions such as health coaching (individual approaches). There exists an opportunity to consider contextual elements which support behavioral change. The practice of “nudging” behavior in primary care and allied health settings is proposed as a means of responding to these contextual opportunities. Nudging does not assure change; however, it can invite curiosity about change and small behavioral efforts in the direction of a desired change. Furthermore, its nature conserves autonomy and patient choice while inviting a health-creating behavior. As such, when considered and applied in the context of public health and individual treatment options, it creates a consistent milieu in which behavior change is facilitated.

Item The ACLM Lift Project—Caring for the Carers(2022-11-15) Renfrew, Melanie; Morton, Darren; Matthews, SimonThe mental health and wellbeing of health care workers has been uniquely challenged by the COVID-19 pandemic. Health care organizations may support the mental health of their members by offering tailored, evidence-based interventions and initiatives. METHODS: The American College of Lifestyle Medicine offered its members an online, 10-week, multicomponent mental health and well-being program (The Lift Project) for free, and extended the invitation to members from sister organizations associated with the Lifestyle Medicine Global Alliance. Individuals (n=1785) representing 39 countries registered for the program. Website meta-data was used to explore participant’ engagement with the program. One hundred forty-five participants responded to a post-program questionnaire that included Likert items and open-ended questions. RESULTS: The website hosting the program recorded 9,815 total visits over 10 weeks. Ninety-one percent of the respondents agreed or strongly agreed that the program supported their mental wellbeing, and there was a significant increase (X2=80.8, p< 0.01) in the proportion of respondents who rated their wellbeing higher after the program compared to at the start of the program. Ninety percent of respondents indicated that the program would have a long-term positive influence on their mental wellbeing, and 94% indicated they would recommend the program to a friend. Dominant themes from the open-ended questions in the post-program evaluation included: the participants liked the style of the video presentations, the structure of the program, content covered and the practical application of the content; the participants would have liked more time to engage with the program and would have enjoyed the provision of group meetings. CONCLUSION: Health care organizations may play an important role in supporting their members’ mental health and wellbeing by offering large-scale, evidence-based interventions. To optimize efficacy, the interventions should be well-designed, engaging, and potentially provide the opportunity for group connection and interaction.

A total of 1785 members registered for the program, representing 39 countries. The website hosting the program recorded 9815 total visits over the 10 weeks. In a post-program questionnaire completed by 145 participants, 91% agreed or strongly agreed that the program supported their mental wellbeing, and participants reported their wellbeing as higher after the program as compared to its commencement (X2 = 80.8, P < .01). Ninety-four percent of respondents indicated they would recommend the program to a friend. Thematic analysis of open-ended responses included in the post-program evaluation indicated that the participants liked: the structure of the program and content covered; the style of the video presentations; and the practical application of the program. Some participants indicated they would have liked more time to engage with the program and would have enjoyed the provision of group meetings. In conclusion, healthcare organizations can support their members’ mental health and wellbeing by offering large-scale, evidence-based interventions.
Item The Practice of Lifestyle Medicine on Physician Wellbeing(2020-01-01) Matthews, Simon; Morton, DarrenItem The Question Cube Re-imagined – A 5-Dimensional Model for Cultivating Coaches’ Capacity for Curious Inquiry in Health Behavior Change(2023-01-01) Matthews, SimonIn a lifestyle medicine context, masterful health coaching requires the development of skills in inquiry. While open inquiry is emphasised, there is a broad range of inquiry types which can elicit narrative responses from a patient. In the 1990s, the ‘Question Cube’1 was conceptualised as a means of teaching therapy students ways to ask insightful questions. The elements of this model were: type of inquiry, subject and orientation. This article updates the Question Cube model to include the original three parameters with the addition of tense and mood. By learning the concepts of inquiry formation, students of health coaching are more able to propose thoughtful and insightful inquiries to their patients.
